Semaglutide

Semaglutide is a Glucagon-like peptide-1 (GLP-1) produced by the gut. It increases insulin production, a hormone that lowers the blood sugar level while inhibiting glucagon secretion, which is a hormone that raises blood sugar, and reduces appetite and energy intake while delaying gastric emptying. It was first developed in 2012 as a longer-acting option for Liraglutide. In 2017 this drug was approved as an anti-diabetic medication used to treat type 2 diabetes and obesity. In June 2021, it got approved for chronic weight management in obese individuals with a (BMI) ≥ 30 kg/m2) or overweight (initial BMI ≥ 27 kg/m2) with at least one weight-related comorbidity. Dosing Information

To reduce adverse side effects, we recommend starting with the dose escalation chart below:

Week 1= 10 units, Week 2 = 20 units, Week 3 = 30, Week 4 = 40 units

Do not exceed 40 units. 20units or 30 units may be the right dose per patient, it usually takes 6-7 months to go up to 40 units.

Tirzepatide/Cyanocobalamin

Tirzepatide Plus is a once-weekly GIP (glucose-dependent insulinotropic polypeptide) receptor and GLP-1 (glucagon-like peptide-1) receptor agonist, representing a new class of medicines studied for the treatment of obesity. Tirzepatide is a single peptide that triggers the body’s receptors for GIP and GLP-1, two natural incretin hormones. Cyanocobalamin will help increase energy.
 
GIP has decreased food intake and increased energy expenditure, resulting in weight reductions. Combined with a GLP-1 receptor agonist, it has a more significant effect on glucose and body weight effects.

Dosing Information

Weeks 1-4 inject 25 units (2.5mg) SQ weekly. Weeks 5+ inject 50 units (5mg) SQ weekly. Anything over 5mg weekly, you would need more than one vial for a month’s supply. The max dose is 15mg weekly which is more for DM2.
